The time and date you select is especially reserved for you. If for any reason you need to cancel or reschedule your appointment, please give at least 24 hours notice to avoid being charged the full session fee.

This form along with the waiver and release form must be signed and completed before any treatment takes place.
Our cancellation/no show policy applies to gift certificate and voucher holders. Gift certificates and vouchers are non-refundable and non-transferable.
Payment is due upon completion of the session unless treatment sessions have been previously purchased .e.g. gift certificate, voucher.
Cash, Visa, MasterCard, Discover and American Express are accepted methods of payment.
A fee of $30 will be charged for returned checks. No refunds are extended for completed services.
Your information will be kept confidential and never be sold.
We regret we cannot extend appointments due to tardiness and you will be responsible for the cost of the appointment. Please be considerate of those appointments following yours by arriving on time. Once you are 20 minutes tardy for your appointment, you are considered a no-show.
